Advance Care Directives

An Advance Care Directive (ACD) is a legal document signed by a person outlining who can make health and wellbeing decisions for them if they do not have capacity to make these decisions for themselves.

The aim of an ACD is to ensure that you have chosen a person you trust to make decisions on your behalf. This is currently the best legal way in South Australia to ensure you are looked after in the manner you planned, if that is required.

Hospitals and care facilties are increasingly requesting these documents from people so the correct person is easily identifiable in the situation where a person cannot make decisions for themselves.
